Consent & Release Form

This agreement is between Michaela Medlin of Dollhouse Ink (Photographer) and,

(Client) for photography services to be provided on the agreed upon date.

We have scheduled for:

A Non-Refundable Deposit of: $50 is due upon the submission of this form in order to secure the agreed upon date and time. This can be completed on the photography page of the Photographers website: www.dollhouseinknv.com

The remaining amount (according to the above chosen option) is due on the day of the shoot by the end of the session.

Cancellation & Rescheduling:

All deposits are non-refundable.

If the Client cancels within 72 hours of the shoot the deposit is forfeited, and a new deposit must be placed in order to reschedule.

In the event of inclement weather or emergencies, the Photographer and Client will make every effort to reschedule for a mutually agreeable date.

The Photographer will provide as many edited, high resolution photos as they can at no extra charge. The final images will be delivered within 2 weeks of the shoot date, via a password protected Online Gallery that will be available to the Client for 2 weeks. Within that time the Client may share the password with family and friends, and all may download as many of the photos as they would like.

The Photographer does not provide unedited or "raw" files.

Usage & Rights:

The Photographer retains copyright ownership of all images. The Client is granted a non-exclusive license for personal use.

Your session fee includes a Personal & Small Business License. You are welcome to use these images for your own website, social media, and personal prints.

However, this does not include "Commercial Resale" or "Third Party Licensing". This means you cannot sell the images to others, and other companies (like a product brand or magazine) cannot use them for their own advertising without my written permisson.

The Photographer may use the images for portfolio, social media, and marketing purposes. The only exception being for Boudoir sessions. If you will be doing a Boudoir or Maternity session and wish for your photos to not be used for portfolio purposes by the Photographer then you must check the box below. (The ONLY photos that would be used are the ones with coverage - fabrics, hands, etc. - no nudity will be posted on social media or the website even with consent.)

If you are a business and will have clients or other employees present in the photos then each person will need to fill out a separate guest consent and release form. (These will be provided the day of the shoot by the Photographer.

The Photographer is not responsible for compromised coverage due to causes beyond their control (e.g. venue restrictions, guest interference, etc.).

Liability & Safety

The Photographer will take every reasonable precaution to perform the services safely. However, the Client is responsible for ensuring the shoot location is safe and clear of hazards. The Photographer is not liable for any loss of business or interruptions that may oocur during the session. The Client agrees to hold the Photographer harmless against any claims for personal injury or property damage sustained during the shoot or during travel to/from the shoot.

If the shoot takes place in the Clients business or home, the Client is responsible for any damage caused to the Photographer's equipment by the Client, their guests, or their pets during the session.

By typing your name below you (The Client) are agreeing to the above contract with the Photographer.
